Thank you for choosing Poetic Moments for creating memories that will last you a lifetime. Below you will find a link to the questionnaire page. Please complete all areas on the questionnaire page as this will assist with the creation and accuracy of your poem. Once the information is received and processed, a poem will be drafted and you will be e-mailed, faxed, or mailed a copy of the first draft for your review.

We ask that you review the first draft carefully for accuracy, spelling, and individual details. After your review, please contact Poetic Moments by phone, e-mail, or mail with your name, order number, and any necessary changes. Once we have implemented any suggestions or changes, your poem will be emailed in final form within 24 hours.
For framed poems, your finished project will be mailed within one (1) week, unless otherwise requested.

Please note that your satisfaction is 100% guaranteed at Poetic Moments. We strive to create the best quality, and if you are not completely satisfied, please notify Poetic Moments within three (3) days of receiving your finished product. We will work with you until we have created a poem that captures your moment as you hoped it would.
